If you are currently pregnant, you will fill out the Prenatal Intake, the Adult New Patient Intake, and the HIPAA Consent. If you are not pregnant, you will fill out the Adult New Patient Intake and the HIPAA Consent. If you are bringing in your child that is 10 years old or younger, you will fill out the Pediatric New Patient Intake (Birth-10 Yrs) and the HIPAA Consent.
